Requesting an appointment does not commit you to surgery. The visit exists to tell you where you actually stand.

Request online
IOI’s request form asks which doctor you’d like to see. Two selections direct your request to Dr. Patel:
- Choose Appointment Request as the subject.
- Under Preferred Doctor, select Neel Patel, MD.
You’ll complete the form on IOI’s website; this site does not receive what you enter there. After you submit, scheduling and follow-up happen through IOI.
What to have nearby
Before you call or open the form, have your contact and insurance information ready, and write down one activity your hip or knee is limiting. Existing patients with clinical questions can use the same number, (317) 620-0232.
